Due to the sensitive nature of our practices, all appointments require completion of a pre-screening interview and waiver paperwork at least two days prior. Once the paperwork is completed, please notify us via email at [email protected] with your proposed date and time of appointment and confirmation that the paperwork has been completed.

Please note, that paperwork and interviews must be completed before an appointment can be scheduled, irrespective of instructor availability.
